How to Join SiOC
Membership in SiOC is very simple.Step 1
Download a Membership Application (Microsoft Word file).
Some agencies require a:
State of California Standard Form # 204, Payee Data Record - (Acrobat PDF file, 1 page, 1.5 megabytes - Note file size!). Use when paying fees at luncheons or yearly membership dues.
Step 2

Completely fill out the form(s).
Step 3

Write a check for the annual membership fee. (Amount is on the application form.)
Step 4

Mail application (and form #204) and check to SiOC.
